1. 01Apply for CareOS as a household / Overseer (click Apply as household).
2. 02Watch for a welcome email from Management after you submit.
3. 03Book an appointment with Management (click Book with Management).
4. 04Wait for Management to accept your household before you pay or sign anything for full onboarding.
5. 05Pay and complete Stripe Checkout only after you are accepted (the invitation or welcome that opens checkout).
6. 06Build and e-sign the CareOS Care Management Engagement with Zoho Sign (current instrument at /contract) when Management sends it.
7. 07Complete the external household intake form after accept for medical power of attorney (required), Living Will (optional), and health details — not in CareOS or Notion. Do not upload these on the public apply form.
8. 08Join Google Workspace when invited — $19 per seat per month for any seated Overseer or Helper (up to 2 Overseers and up to 2 patients per account). Emergency Contacts and Providers may be Workspace guests. They are not paid $19 seats and not Overseer or Helper accounts, and they have no full portal accounts.
9. 09Await your Operator to send invites to Notion AI and Slack.
10. 10Choose the BaaS fee / Stripe Issuing card program if you want that hosted card rail instead of your own prepaid card.Add-on

You will not be asked to check out or pay until Management accepts your application.

They are not paid $19 seats and not Overseer or Helper accounts, and they have no full portal accounts. Notion holds non-health care logistics (Open Loops). A medical power of attorney is required. A Living Will is optional. After Meridian Business & Civic Group accepts the household, the Operator sends the household intake link for medical power of attorney, Living Will, and other health details (Google Form under Workspace BAA) — not Notion, not email, and not as PHI fields in CareOS. Intake form link coming soon — the Operator will send it to you directly. The September 20, 2026 “Overseer portal only” upload lock is superseded for public legal pages.
